ABOUT THIS ROLE

<p>The Caregiver Support Program Clinical Assessor, Registered Nurse, is responsible for knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day-to-day operation of the Caregiver Support Program (CSP), and all reporting requirements within the assigned Veterans Integrated Systems Network (VISN).</p><h3>Major Duties</h3><p>The Caregiver Support Program Clinical Assessor, Registered Nurse, Critical Care, duties include, but are not limited to the following: Assesses, plans, implements and evaluates care based on age-specific components. Delivers fundamental knowledge-based care and counseling to assigned clients while developing technical skills. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate beginning leadership skills and participates in specific PCAFC experiences. They demonstrate creative approaches to the management of aspects of complex patient care. Provides services of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling. Possesses thorough knowledge of PCAFC, PGCSS, clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure Veterans and caregivers are provided with thorough, comprehensive care options within the scope of the CSP. Virtual: This is not a virtual position.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
