Registered Nurse (Care Manager)

<p>As a team member of the HUD-VASH program, the HUD-VASH Registered Nurse Care Manager is a member of the HUD-VASH program and as such will provide support to the HUD-VASH teams and serve in a medical and mental health consultative role. The incumbent consults with the Lead HUD-VASH case manager, the HUD-VASH teams, the Medical Director for the Homeless Patient Aligned Care Team, the VISN Homeless Coordinator and other colleagues as needed to best meet the treatment needs of the Veteran.</p><h3>Major Duties</h3><p>Major duties and responsibilities include, but not limited to: In consultation with other healthcare providers (Primary care and outpatient Mental Health), the nurse provides coordination and monitoring of outpatient treatment methods. The HUD-VASH RN Care Manager provides comprehensive nursing care, including psychosocial assessment, implementation of treatment, referral to appropriate programs and providers and comprehensive case management, and home visits to veterans with physical and mental health issues who face challenges in finding and maintaining stable housing. This nursing position requires a balance of clinical and administrative skills that will allow the incumbent to function with maximum efficiency in high volume multitask work environment. The goal of program and care interventions is to assist in efforts to move Veterans and their families out of chronic homelessness. Provides patient and family health education with a focus on self management, prevention, and wellness, based on the patient's goals. Partners with other high risk outpatient clinics (standardized care management of high-risk patients).
