ABOUT THIS ROLE
<p>Demonstrates proficiency in clinical practice through coordinating care, prioritizing patient education, self-management, and customer satisfaction across the entire continuum of care. Diligently follows established procedures, policies, and guidelines to maintain safety and quality standards. Consistently collaborates with members of the interdisciplinary team, effectively influencing patient outcomes through teamwork and communication.</p><h3>Major Duties</h3><p>The Care Coordinator Registered Nurse (RN) is responsible for providing high-quality, customer-focused, and cost-effective patient care as it relates to his/her role within the Gastroenterology Service. The incumbent maintains a strong foundational nursing knowledge and advanced clinical expertise in gastroenterological conditions and interventional procedures. The Care Coordinator assists Veterans, family members, and caregivers in accessing appropriate services and resources to address their complex health care needs. The RN delivers evidence-based nursing care and oversees the coordination of care for Veterans receiving GI procedures and services in the ambulatory setting. Key responsibilities include facilitating appointment planning, care navigation, patient education, and communication among providers to ensure timely evaluation, treatment, and follow-up. Additional duties involve monitoring diagnostic results, supporting care transitions, addressing clinical and psychosocial needs, and promoting continuity of care through collaboration with the interdisciplinary team. The incumbent demonstrates leadership in delivering and improving holistic care through collaborative strategies, continuously evaluates practice based on best evidence, contributes a professional nursing perspective within interdisciplinary discussions, partners with colleagues to drive improvements in patient outcomes, and shares knowledge to support the delivery of safe, high-quality nursing care.
