ABOUT THIS ROLE

<p>The position is for a Registered Nurse Care Coordinator for Nursing Operations Service for VA Long Beach Healthcare System located in Long Beach, California.</p><h3>Major Duties</h3><p>The RN practices in a complex patient care setting in collaboration with colleagues and Nurse Managers with minimal supervision. The RN is accountable as assigned as a primary nurse or a team leader for a defined group of patients on his/her shift or for an entire episode of care. Based upon education and experience, this nurse may provide direct supervision and be assigned as a preceptor or shift charge nurse.Other duties include, but are not limited to: Provides a full range of nursing care to patients with complex care needs utilizing the nursing process based on the ANA Standards of Nursing Practice. Demonstrates consistent clinical competence in providing nursing care. Uses equipment correctly and accurately interprets data. Recognizes subtle changes in patient's condition and responds timely, choosing the correct interventions. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit. Performs ongoing assessment of patient condition and response to treatment. Assess the patient's learning needs, learning preference, and readiness to learn. Documents the nursing process and is consistent, accurate, concise, and timely. Revises plan of care according to evaluations of patient responses. Accurately documents in the medical record patient findings, assessments, and care provided. Uses the Computerized Patient Record System (CPRS) and the Bar Coded Medication Administration (BCMA) software per policy. Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes. Demonstrates grade-appropriate and consistent critical thinking skills. Role-specific Competency Checklist is complete and is rated competent in all measured areas. Preferred Experience: 1-2 years related Care Coordinator experience.
