ABOUT THIS ROLE
<p>The Cardiac Medicine Registered Nurse Coordinator provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. This is an Open Continuous Announcement and will remain open until September 30,2026. Qualified applicants will be considered and referred as vacancies become available. Announcement may be closed earlier if no longer needed.</p><h3>Major Duties</h3><p>Duties include but are not limited to: Triaging Cardiology Consults to determine appropriate access. Working with patients and providers to determine plan of care and treatment strategy. Providing cardiology specific information / education / instruction. Evaluating care and outcomes to ensure timely and appropriate provision of services. Establishing a therapeutic relationship, allowing the patient to attain, maintain or regain optimal function through assessment and treatment. Engaging patients, families, and other caregivers to incorporate knowledge, values, and beliefs into care planning without judgement or discrimination. Following established policies of the practice setting, VA, and ANA Code of Ethics for Nurses. Participating in unit-based activities to improve and deliver cost effective patient care. Protecting printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy, and VHA policy. Work Schedule: Monday through Friday, 07:30AM to 4:00PM OR 07:30AM to 6:00PM compressed work schedule (days to be determined). Compressed/Flexible: Available. Telework: Not authorized. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Note: In order to grant credit towards an appointment based on foreign nursing education higher than the associate degree level, formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD) must be obtained.
