ABOUT THIS ROLE
<p>The Cancer Care Program Coordinator (CCPC) is a Registered Nurse whose primary responsibility is to lead the Cancer Program by reviewing, assessing, and measuring outcomes related to the process involved in identifying cancer to include all modalities for cancer from the initial screenings through diagnostic studies.</p><h3>Major Duties</h3><p>Major duties include, but are not limited to the following: Develops and implements the Cancer Program and ensures compliance and collaboration within the local facility and all VA Central Office Cancer Program initiatives. Maintains a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences. Prioritize responsibilities and collaborate with other professions and disciplines to ensuring optimal outcomes from a program level. Collaborates with primary care clinicians to provide Veteran access to proper care indicated by screening guidelines. Stays abreast of current federal and state legislation directed at colorectal cancer screening to continue to provide safe effective care to the Veteran. Collaborates with the Colorectal Cancer Review Committee to establish protocols relevant to the issues of colorectal cancer screening. Coordinates and expedites all points of care from screening tests to definitive treatment/surgical intervention, if needed. Collaborates with the oncology and surgical services, when appropriate, to transition care of the Veteran for treatment options. Reviews all positive cancer screens on an ongoing basis for clinical appropriateness, co-morbidities and care in the community and ensures that patients are educated about treatment options if appropriate. Initiates quality improvement activities that result in improved care and conducts ongoing data collection to track and improve the Cancer Program. Other duties as assigned.
