<p>Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.</p><h3>Major Duties</h3><p>The primary responsibility of the Registered Nurse - Survivorship Care Coordinator is to coordinate the journey of the veteran patient with cancer through the health care system. The incumbent of this position is essential to the successful operation and outcomes of the cancer treatments received in the Hematology/Oncology Department. The incumbents' activities are dedicated to the timely access to quality care and treatment for patients with cancer which increases patient safety and satisfaction. The nurse is a stable and reliable point of contact for Veteran patients, providers, and member of the clinical team. The incumbent provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, and completed timely. Incumbent is responsible for tracking the overall treatment plan while following select patients from the earliest suspicion of cancer through the continuum of cancer care inclusive of diagnosis, treatment, survivorship/surveillance and or palliation/end of life. Typical procedures include PET scanning or other designated Nuclear Medicine procedures, radiology or laboratory studies; pre-operative diagnostic studies coordinated with clinical subspecialties such as Surgery, Cardiology or Dental Service; and /or facilitating clinic appointments, coordination and arranging diagnostic endoscopies or other invasive procedures. VA offers a comprehensive total rewards package: VA Nurse Total Rewards Major duties and responsibilities include, but are not limited to: Provides direct patient care to Ambulatory Infusion Patients during staffing shortages in a primarily administrative role. Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, and must provide direct patient care in Ambulatory Infusion Clinic when short staffing occurs among regular AIC nursing staff. Uses advanced clinical knowledge/judgement to promote staff involvement in planning, decision- making, and evaluation outcomes. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluates current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise. Guides, develops, and supports staff from a leadership perspective. Uses professional standards of care, scientific evidence, and practice to evaluate programs and /or service activities. Accounts for utilizing critical thinking skills and sound judgement to ensure positive outcomes, maximizing third party collections and delivering quality patient care performance improvement activities. Facilitates seamless transition through multidisciplinary/departments throughout veterans' course of treatment. Demonstrate knowledge and ability to apply evidence-based practice to plan of care. Provides support at the unit/service and organizational level. Coordinates with laboratory and assisting in bone marrow biopsies; providing patient education ensuring informed consent and providing oversight to ensure completion of require clinical and/or administrative documentation in accordance with (VA) medical center policies and Joint Commission Standards. Serves as consultant for problem solving in nursing and organizational issues.
