<p>The Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to Ambulatory Care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Ambulatory Care Department.</p><h3>Major Duties</h3><p>The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse - assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The PACT Outpatient Staff RN is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. The PACT RN provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. This nurse integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations. Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. The PACT RN validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The nurse will demonstrate the ability to cope with and manage competing priorities. The nurse demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills. Other duties as assigned. Delegate and supervise basic nursing care of licensed practical nurses and unlicensed assistive personnel. Provides urgent care to patients with acute and/or chronic medical, surgical, and psychiatric conditions. Ability to formulate a plan of care based on nursing assessments. Seeks guidance in how to improve care provided to patients and families. Assess the patient by collecting pertinent data or information, including the triage of patients based on the five-level triage system to prioritize patients that require more immediate care. Identify expected outcomes to develop a collaborative and individualized plan for each patient. Implement that plan, including care coordination, health teaching, and health promotion. Evaluate the progress toward achieving the expected goal and outcomes and adjusts the plan as necessary.
