ABOUT THIS ROLE
<p>The RCI Nurse will be a part of the SFVAMC Referral Coordination Team (RCT) with Community Care. As part of the RCT RN will help to streamline the referral and consult management process by ensuring timely care delivery and helping Veterans make more informed decisions about where to receive their health care. The RCI RN will be responsible for identifying community care eligibility and having a meaningful discussion with Veterans about care modalities both within the VHA and in the community.</p><h3>Major Duties</h3><p>Referral Care Coordination (RCI) Registered Nurse duties include: Collaborats with physicians on clinical reviews, keeps them appraised of care options available locally. Reviews utilization patterns, identifies trends and problem areas for special studies. Assists in collecting and assimilating clinical data to enhance the quality of services. Generates quality improvement results. Reports and investigates unusual occurrences and questions inappropriate decisions based on their professional expertise. Serves as a change agent across business initiatives and programs, in partnership with internal and external stakeholders. Leads complex projects and programs, from ideation through to cost/benefit results tracking, utilizing program management and business process management best practices. Provides clinical expertise and clinical leadership in the overall case management of all complex patients throughout the continuum of care. Provides Veteran-centric care by providing health care options to Veterans to receive their care at a VA medical facility or though community care providers when eligible. Collaborates with providers to implement plans of care and assures follow-up interventions to meet critical needs and positive outcomes. Implements a quality and case-management program, with a focus on performance measures and meeting goals. Provides direct nursing consultation, nursing clinical leadership and serves to assure continuity of care. Shares and works with all staff, responsible for day to day nursing schedule and any changes/updates. Assists Manager in the responsibility for yearly and time sensitive staff proficiencies and competencies. Helps ensure continuity of care and continuum of care for all patients met San Francisco VA System. Provides education and training to other clinical areas as needed with regards to RCI. Supports the veteran and family on continuous Care. Manages case activities and telephone triage from patients and providers. Precepts and trains nursing student's new nurses and other staff throughout the VA system. Other duties as assigned.
