<p>The Cincinnati VHA Medical Center is seeking a Primary Care PACT RN (Patient Aligned Care Team) at the Hamilton CBOC Clinic. This RN is responsible for providing competent, evidence-based sex-specific care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Major Duties may include but are not limited too: Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. Integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. Demonstrates the ability to cope with and manage competing priorities. Demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills. All other duties as assigned. Preferred Experience: Previous Primary Care or Ambulatory Experience. Supervisory Controls: The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).
