<p>The Telehealth and Mental Health LPN Telehealth SFT Preceptor is responsible to the Nurse Manager or assigned supervisor and is accountable to the Associate Director of Patient Care Services (ADPCS). Meets the needs of all customers while supporting the Medical Center and Service missions. Consistently communicates and treats customers (patients, visitors, volunteers, and all Medical Center staff) in a courteous, tactful, and respectful manner.</p><h3>Major Duties</h3><p>The LPN is responsible for the day-to-day operation of the Imaging Clinic at the parent facility and associated CBOCs. The LPN performs ocular imaging for patients who are candidates for Teleretinal Imaging as identified and referred by the VA Primary Care Provider, VA eye care providers, and the LPN. Guided by local policy/standing orders, the LPN can initiate consultations for patients on behalf of the PCP, who retains responsibility for patient care. The LPN serves as the care coordinator Teleretinal Imager. The LPN successfully and consistently demonstrates exercise of independent technical judgment as well as comprehensive and thorough working knowledge of the most complex or difficult practices and procedures. The LPN completes an abbreviated history including: duration of diabetes, insulin/oral/dietary control, last hemoglobin A1C value, last dilated eye exam by date and provider, past ocular history, family ocular history, and current ocular medications. Demonstrates the ability to provide a full range of care to patients who require complex treatments/care. Administers medications accurately with the remote supervision of a registered nurse (RN).- Exhibits the capability to assist the RN and/or the provider to aptly prioritize overall patient care needs and modify care plans as appropriate. Practices patient centered care. Readily performs personal care, diagnostic support duties and treatments according to policy/procedures under the direction of an RN. Responds to patient's needs, including physical comfort and emotional needs, and recognizes deviations from the normal. Possesses the independent, complex, and technical knowledge and skills that support the successful completion of the Telehealth visit. Maintains a current knowledge of Telehealth program requirements. Work Schedule: Full Time,Mon-Fri 0800am-4:30pm. Duty Location: TBD during interview/selection process.
