<p>The Licensed Practical Nurse (LPN) is responsible for the delivery of care to Veterans in the primary care clinic. The LPN is responsible for the treatment, observation, and care of the Veteran within the scope of practice to assist in the overall PACT clinic patient flow, procedures, the check in and checkout process, scrubbing schedules, and running daily clinic with the Provider. The LPN provides direct and indirect nursing care to patients and families within the clinic.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional The LPN is a key member of the PACT and multidisciplinary team and is responsible for collaboration with other team members, promoting effective communication and ethical practice while integrating population health to meet the needs of the complex Veteran. The LPN uses a systematic problem-solving process in the care of patients with chronic health care needs to provide individualized, goal-directed nursing care. The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family. Duties include but are not limited to: Uses motivational interviewing and health coaching to guide patients Scrubs future and daily schedules for patient care needs Provide medication list for patient review Performs vital signs and treatments Assists providers with exams/procedures Completes appropriate clinical reminders Administers/tracks immunizations Tracks, prepares, and administers medications Completes orders per provider or nursing protocol Facilitates visit closure by confirming required patient education and orders have been processed/completed Observes, screens, records, and reports patient's response or changes in patient's condition Promptly recognizes and responds to urgent or emergent situations Serves as preceptor for nursing orientation Places orders and consults per clinical reminders and PACT Nurse Protocols working to the top of education and competency Checks daily equipment/supplies and restocks Responds to patient calls, messages, and alerts Completes primary care discharge phone calls per clinic policy Obtain labs and other specimen collections Collaborate with PACT staff to develop comprehensive health care plans for patients assigned to patient panels Manages clinic workflow and daily schedule. Ensuring patients are placed in examination rooms in a timely manner, and providing direction to patients as they move through the clinic environment; runs the daily clinic with the Provider Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients Recognizes urgent or emergent patient care situations, seeks assistance of the RN and/or provider, and initiates appropriate emergency interventions as directed Completes nursing visits working to the top of education and competency Performs other duties as assigned Work Schedule: Monday - Friday 7:00am 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
