Physician (Internal Medicine)

<p>About the Position: This position is located at Brooke Army Medical Center in Fort Sam Houston, San Antonio TX. This is a Direct Hire Solicitation. Recruitment or relocation incentives may be authorized for highly qualified candidates. Salary negotiation may be available for those candidates who are new to Federal service.</p><h3>Major Duties</h3><p>Serve as an Internal Medicine Physician in a Patient Centered Medical Home (PCMH); scope includes a broad spectrum of health services for adults ranging from preventative medicine to diagnosis and treatment of complex acute and chronic diseases.</p><p>Provide first contact, continuous, comprehensive primary care for adults and DoD beneficiaries according to standards of care, the PCMH model of care, Army policy, and relevant regulations.</p><p>Coordinate and/or integrate care across all elements of the healthcare system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient's community; Admit patients to the hospital when appropriate.</p><p>Lead and provide technical oversight of multidisciplinary clinical team (e.g. nurses, social workers, therapists, and various technicians) that are assigned to the medical home and responsible for panel management and patient care.</p><p>Teach and provide consultation to other health care providers as assigned.</p><p>Complete required documentation and records pertaining to all medical services provided; properly safeguards all reports and records according to the Health Information Portability and Accountability Act (HIPAA) standards.</p><p>Participate in Continuing Medical Education and quality assurance activities.</p><h3>Education</h3>See Qualifications section for education requirements.
