<p>Provides the primary clinical treatment to patients of the medical center. Recruitment/Relocation Incentive (Sign-on Bonus): Authorized for Highly Qualified Applicants.</p><h3>Major Duties</h3><p>The Primary Care PA will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the CBOC and the WNCVAHCS. This individual is delegated the authority and responsibility in his/her area of expertise to assume major clinical, consultative, educational, research, and administrative duties and assignments related to the examination of claimants filing compensation and pension claims. VA offers a comprehensive total rewards package. Clinical: The Primary Care PA's Clinical responsibilities will include, but not be limited to, the following: i. Demonstrates the ability to independently manage a panel of patients with complex, chronic care issues using the best available evidence and national guidelines to achieve optimal population health and effective disease management. ii. Demonstrates leadership and works collaboratively with assigned Primary Care Team and other interdisciplinary teams to identify, analyze and coordinate internal and external resources to resolve care issues and meet the medical and psychosocial needs of assigned panel of patients. iii. Appropriately orders diagnostic and therapeutic tests relative to patient's age-specific needs and health condition. iv. Appropriately prescribes pharmacologic and non-pharmacologic interventions that support the patient's ability to maintain or regain their health status. v. Monitors and documents the effectiveness of treatment interventions, achievement of institutional quality measures and performance metrics. vi. Initiates referrals/consults to the appropriate service when indicated vii. Sets clinical care goals, short and long term, in collaboration with patient, provider(s), and family members/caregivers. viii. Provides education to patients and their families regarding the complex medical, social, and behavioral conditions with a focus on self-management, prevention and wellness based on the patient's health goals and needs. ix. Administrative: i. May review in coming requests for new patient examinations to ensure adequate resources are available to complete these requests. ii. Works closely with the Chief of Primary care and nurse managers to complete evaluations in a timely manner withdetail to quality. iii. Assists with arranging continuity care across the VHA system as needed. iv. Works to provide leadership on site to ensure care is completed in required timeframes. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
