Physician Assistant - Home Based Primary Care

<p>The Physician Assistant (PA) practices in Home Based Primary Care Service (HBPC) to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The PA provides advanced clinical practice, consultation, and management. NOTE: The 2-page Resume requirement does not apply to this position. Duties include but not limited to: The Physician Assistant (PA) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice in the patient's home. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Home Based Primary Care (HBPC) interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Determines the need for and facilitating patient admission to the VA medical facility or other transitions of care. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP), which authorizes VHA to buy out service contracts (Locum agreements, loan repayments, sign-on bonus contracts, Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years).
