<p>The Physician Assistant (PA) practices in Home Based Primary Care Service (HBPC) to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The PA provides advanced clinical practice, consultation, and management. NOTE: The 2-page Resume requirement does not apply to this position. For more information, refer to Required Documents below.</p><h3>Major Duties</h3><p>VA offers a comprehensive total rewards package. Duties include but not limited to: The Physician Assistant (PA) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice in the patient's home. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Home Based Primary Care (HBPC) interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Determines the need for and facilitating patient admission to the VA medical facility or other transitions of care. Work Schedule: Monday-Friday, 8:00am-4:30pm Compressed/Flexible: May be available and will be discussed upon selection Telework: Available on an as needed (Ad-Hoc) basis as determined by the supervisor Virtual: This is not a virtual position. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP), which authorizes VHA to buy out service contracts (Locum agreements, loan repayments, sign-on bonus contracts, Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years). Open to U.S. Citizens but non-citizens may be appointed when it is not possible to recruit qualified citizens. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Must be proficient in written and spoken English. Subject to background/security investigation. Selected applicants will be required to complete an online onboarding process. Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents). Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
