<p>The Physician Assistant practices in the Community Living Center to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The Physician Assistant provides advanced clinical practice, consultation, and management.</p><h3>Major Duties</h3><p>Experience the welcoming spirit of Sheridan, Wyoming-where historic downtown charm and Western legends meet the spectacular beauty of the Bighorn Mountains. Since 1922, the Sheridan VA Medical Center (VAMC) has been dedicated to serving Veterans, stemming from its origins as a historic military fort. Our center has grown from a 300-bed hospital to a modern 208-bed facility that proudly serves nearly 12,500 Veterans each year. With eight Community Based Outpatient Clinics throughout Wyoming-including Casper, Gillette, Riverton, Rock Springs, Powell, Afton/Star Valley, Evanston, and Worland-the Sheridan VAMC delivers comprehensive primary and mental health care across the Rocky Mountain region. VA offers a comprehensive total rewards package. Duties include, but are not limited to: -Obtains medical history send perform physical examinations and admission to CLC and annually. -Documents discharge note and coordinates discharge care for veterans including medication reconciliation, follow-up appointments, consultations and coordination with other team members, in accordance with VHA standards. -Orders and interprets diagnostic tests, orders therapeutic procedures such as lab tests, ECG, radiological studies, oxygen therapy, etc., based on current CLC treatment guidelines. -Response to medical emergencies for CLC patient's and coordinates transfer to higher level of care when necessary. - Orders VA specialty service consults and initiates request from non-VA medical care and consults and accordingly VHA policy and local facility availability. Orders interfacility transfers and other transportation for CLC veterans. - Orders are prescribed devices, supplies, and durable medical equipment. - Orders were prescribed medications for patients for their every day care. - Conduct goals of care conversations with high risk patients regarding any desired limitation and life-sustaining treatments and choice of surrogate and then enters life-sustaining treatment (LST) progress note and corresponding orders. - Provides clinical oversight and participates in peer review other community living center providers through periodic chart reviews as needed. - Acts as a surrogate for absent CLC providers as needed to include review of EMR view alerts and other duties as assigned. Participates in team coverage for vacations, sick days, etc. - Regularly attends all provider meetings, departmental meetings, and medical staff meetings as requested - Participates in formulating multidisciplinary treatment plans. - Provided health education to patient's and other team members. - Coordinates with CLC admissions team, determining appropriateness for admissions and discharges from the CLC. - Refers patient's to appropriate VA or community programs. - Other necessary duties as reasonably assigned by supervisor within the scope of practice of the provider. Work Schedule: 0700-1530 Monday through Friday Compressed/Flexible: Yes, compressed schedules are available. Telework: Not Available other than weather and safety leave. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP), which authorizes VHA to buy out service contracts (Locum agreements, loan repayments, sign-on bonus contracts, Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years). Open to U.S. Citizens but non-citizens may be appointed when it is not possible to recruit qualified citizens. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Must be proficient in written and spoken English. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
