<p>The duties and responsibilities of the Pharmacist position require completion of a pharmacy specialty residency (PGY-2) in Infectious Disease. The incumbent functions as a clinical pharmacist, providing clinical pharmacy services to providers in support of inpatient and outpatient infectious diseases and the patients under their care. The incumbent is responsible for planning and coordinating educational programs related to pharmacotherapy in Infectious Disease & HIV Medicine for pharmacy staff.</p><h3>Major Duties</h3><p>VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Assists providers with antibiotic and drug interaction issues for patients cared for by infectious diseases specialists and trainees. Directs patient education, with focus on patients with literacy barriers, and patients who request more in-depth medication problems. Provides clinical pharmacy services in outpatient infectious diseases clinics (including the HIV clinic). Coordinates conferences for infectious diseases trainees and HIV care providers as appropriate. Participates in ID consult team as assigned. Completes training as necessary for becoming more knowledgeable in areas of clinical pharmacy, research, or quality improvement projects. Assists in maintenance of vaccination database and setting appropriate clinical reminders for vaccinations. Evaluates and responds to prior authorization drug requests, using national formulary and local criteria, within 96 hours. Participates in initiatives to improve vaccine compliance and integrates new vaccines into medical center practice as appropriate. Maintains a safe and clean work environment free of any safety and/or fire hazards. Reads and is knowledgeable of all Medical Center and Pharmacy Service Fire and Safety Policies.
