Outpatient Registered Nurse- Patient Aligned Care Team

<p>Ambulatory Care is seeking a skilled and motivated Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) to join our team. In this role, you will provide high-quality, evidence-based, gender-specific nursing care to Veterans in an outpatient setting. You will play a key part in supporting Veterans' overall health and wellness by delivering direct patient care, coordinating services across the care team, and ensuring a positive, seamless experience for those we serve.</p><h3>Major Duties</h3><p>In this role, the PACT Outpatient Staff Registered Nurse (RN) provides compassionate, high-quality nursing care to Veterans within the Ambulatory Care Service. The nurse delivers direct patient care, coordinates services across the care team, and supports Veterans in understanding their health conditions through education, wellness coaching, and disease-prevention guidance. The RN plays an essential role in helping Veterans manage chronic illnesses by assisting with goal setting, monitoring progress, and staying engaged in their care plans. The nurse collaborates closely with physicians, nurse practitioners, social workers, and other team members to ensure seamless, holistic, and Veteran-centered care. The position also contributes to quality-improvement efforts, supports training and competency development for new staff, and fosters a positive, professional team environment by offering constructive feedback and sharing clinical knowledge. Throughout daily practice, the RN uses strong critical-thinking skills to respond to changing needs, prioritize multiple tasks, and apply up-to-date, evidence-based clinical practices. Additional duties may be assigned to support the clinic's mission and ensure exceptional service to Veterans.
