<p>The purpose of this position is to help plan, coordinate, implement, and evaluate community-based nutrition care, education, and promotion of services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).</p><h3>Major Duties</h3><p>Provide consultation and technical assistance to the Clinical Diabetes Program, Health Promotion Department, and other professional staff to help coordinate service unit wellness programs and other diabetes prevention programs. Help review and assess program progress and operations to identify and make recommendations for improving the quality and assess the appropriateness of community nutrition services and programs. Conduct nutritional assessment of clients, developing nutritional care plans with them and their families, consistent with cultural, social, and physiological requirements. Record information in medical records (progress notes) regarding nutritional requirements, patient progress, education, follow-up, etc. Report and summarize progress and activities at regular intervals and prepare periodic and special reports or projects.</p><h3>Requirements</h3>Selectee may be subject to a probationary period ESEP appointees typically serve a two year trial period U.S. Citizenship is required Selective Service Registration is required for males born after 12/31/1959 This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children. Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment. A background investigation is a standard process used by the Federal Government to verify an individual's identity, employment history, education, and other relevant information to determine suitability for Federal service.
