ABOUT THIS ROLE
<p>Fredericksburg HCC is recruiting for Registered Nurse (RN) -Surgical Nurse Navigator. This position is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. Serves to support delivery of high quality, timely care to the Veteran receiving services in the community. This position is located in the Outpatient Surgery Clinic, Fredericksburg HCC, Fredericksburg Virginia.</p><h3>Major Duties</h3><p>Duties include but are not limited to: Participates in the unit/program level quality improvement process and initiatives as well as customer service programs. Complies with VA-mandated processes and use of health information solutions that serve to assess, document, exchange, and track care coordination information. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Excellent customer service skills, verbal and writing skills. Ability to adapt to frequent change, and multi-tasking. Surgical Nurse Navigator should possess recent clinical/case management/care coordination/utilization management skills or a combination of all. Must be detail oriented, and have problem-solving and researching skills Excellent organizational skills and ability to conduct clinical chart reviews, review of medical records.
