<p>The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.</p><h3>Major Duties</h3><p>Located in the heart of Oregon's high desert, Hines offers a welcoming small-town atmosphere surrounded by stunning natural landscapes. General Description of Assigned Duties include, but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Coordinates care with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures in accordance with established policies and guidelines. Influences patient care outcomes by collaborating with members of the interdisciplinary team. Executes responsibilities that demonstrate leadership, experience, and creative approaches to managing complex client care. Functions or Scope of Assigned Duties include, but are not limited to: Manages assigned patient panel(s), utilizes registries, and ensures appropriate evaluation and access for all patients within the panel. Provides health promotion, health education, and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines. Demonstrates leadership performance broad enough to improve care delivery for the Primary Care Department and the facility. Provides peers with formal or informal constructive feedback to support continuous improvement. Supports colleagues and other nurses through primary care knowledge sharing to promote safe, high-quality nursing care. Fosters a safe and supportive environment that encourages the professional development of healthcare staff. Evaluates outpatient outcomes of evidence-based decisions and practice changes for individuals, groups, and populations within primary care. Responsible for documented Patient Aligned Care Team (PACT) outcomes at the program or service level. Ensures that PACT program or service-level outcomes are broad, complex, and demonstrable at any organizational level within the facility. Permanent Change of Station (PCS): Not authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
