Nurse Manager - Homeless Patient Aligned Care Team

The Homeless Patient Aligned Care Team (HPACT) Supervisor is a registered professional nurse who is accountable for the management of patient care in an outpatient clinic setting. This position is located in Social Work Services at the VA Salt Lake City Health Care System, Salt Lake City, Utah and provides services within the program both on site and in the community. Provides primary care services under two programs, Homeless-Patient-Aligned Care Team (HPACT) and Project-Based Geriatric Team.
