Nurse (Disease Management)

<p>About the Position: This position is located at Laughlin Air Force Base, Texas. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes. Payment of Permanent Change of Station (PCS) costs may be authorized.</p><h3>Major Duties</h3><p>Provide comprehensive Disease Management Services such as assessing patient care needs, providing patient health management education, and monitoring patient progress.</p><p>Collaborate with providers and other medical professionals to develop treatment programs and coordinate services for low and high-risk patients with diseases/conditions across the healthcare continuum.</p><p>Utilize a variety of specialized medical equipment to carry out clinical or diagnostic procedures.</p><p>Observe patient vital signs, monitoring for adverse reactions and providing appropriate emergency care as needed.</p><p>Write clinic policies and procedures by participating in Quality Improvement and Risk Management activities, health maintenance programs, and collecting data on programs.</p><p>Train healthcare teams on the purpose of disease management, population health, and preventative services.</p><h3>Education</h3>See Qualifications section for education requirements.
