Nurse (Disease Management)

<p>About the Position: This position is located at the 20th Medical Group, Shaw Air Force Base, South Carolina. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.</p><h3>Major Duties</h3><p>Provide comprehensive Disease Management (DM) Services.</p><p>Develop treatment programs to provide advanced level, specialized, and comprehensive professional nursing care.</p><p>Perform clinical or diagnostic procedures.</p><p>Provide nursing support to clinic functions during base operational readiness exercises/inspections or major events requiring healthcare assets.</p><p>Collaborate with healthcare professionals/departments to identify members needing additional education or clinical management of selected diseases.</p><p>Generate data through the use of assessments, screening, and other data tools to identify individuals with specific disease states/conditions/or risk.</p><p>Orient healthcare teams to the purpose of disease management activities, population health, clinical preventive services, and medical management.</p><h3>Education</h3>See Qualifications section for education requirements.
