Nurse (Clinical/Case Management)

<p>About the Position: This position is located at Army Health Clinic Munson, Fort Leavenworth, Kansas. This is a Direct Hire Solicitation A recruitment incentive may be authorized. Non-Federal work experience may be creditable for leave purposes. Salary negotiation may be available for those candidates who are new to Federal service.</p><h3>Major Duties</h3><p>Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.</p><p>Analyze complex medical, psychosocial, and administrative problems to recommend effective solutions and improve outcomes.</p><p>Provide comprehensive assessments of patient health needs and develop individualized, cost-effective care plans.</p><p>Evaluate patient progress through follow-up assessments, documenting interventions and guiding informed decision-making.</p><p>Collaborate with multidisciplinary teams and serve as patient advocate to minimize barriers and optimize quality of life.</p><h3>Education</h3>See Qualifications section above.
