ABOUT THIS ROLE
<p>About the Position: This position is located at Madigan Army Medical Center Joint Base Lewis McChord, Care Management Services, working as a Case Manager in the Madigan Internal Medicine Clinic The work involves collaborating with interdisciplinary health care team and the patient/caregiver to address areas of concern, guide informed decision making, and encourage the patient on quality-of-life improvement. This is a Direct Hire Solicitation</p><h3>Major Duties</h3><p>Will serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).</p><p>Conduct comprehensive assessments of beneficiaries’ health status, functional limitations, and support systems to develop individualized plans of care.</p><p>Collaborate with providers and support staff to address these variances and recommend adjustments that improve care efficiency and patient outcomes.</p><p>Gather data from multiple healthcare agencies, document patterns affecting quality of care, and escalate concerns that may indicate system-level issues.</p><p>Educate patients, families, and healthcare team members on treatment options, community resources, and case-management processes to support informed decision-making</p><p>Review referrals for accuracy, assist providers in directing patients to appropriate specialty services</p><p>Screen potential case-management clients within established timeframes to determine eligibility and benefit</p><p>Document supports quality improvement by ensuring complete, timely, and accurate records that reflect patient progress, barriers to care, and outcomes.</p><h3>Education</h3>FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
