Nurse (Clinical/Case Management)

<p>About the Position: This is a Nurse (Clinical/Case Manager) position at Fort Drum, New York. This is a Direct Hire Solicitation. Salary negotiation is available for those new to Federal employment. Recruitment, Relocation and/or Student Loan Repayment may be authorized.</p><h3>Major Duties</h3><p>Serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).</p><p>Conduct comprehensive assessments of beneficiary’s health needs to develop a plan of care.</p><p>Educate both patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that informed decisions can be made.</p><p>Consult and coordinates with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems.</p><p>Serve as a liaison between the patent and the health care professionals.</p><p>Update follow up assessment information with the patient's physical and emotional condition within their electronic record.</p><p>Address current issues and improve overall processes by contributing to daily multi-disciplinary team meetings.</p><h3>Education</h3>FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
