Nurse (Clinical/Case Management)

<p>About the Position: This position is located in the Internal Medicine Clinic on the 2nd floor (main entrance of the hospital) in a clinic environment at Dwight D. Eisenhower Army Medical Center, Fort Gordon, GA. This position serves as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF). Clinic hours are 0730 - 1600 hrs. This is a Direct Hire Solicitation.</p><h3>Major Duties</h3><p>Serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).</p><p>Conduct comprehensive assessments of beneficiary’s health needs to develop a plan of care.</p><p>Educate both patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that informed decisions can be made.</p><p>Consult and coordinate with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems.</p><p>Serve as a liaison between the patient and the health care professionals.</p><h3>Education</h3>FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
