Nurse (Clinical/Case Management)

<p>About the Position: This position is located at 22ND Medical Group McConnell Air Force Base, Kansas. This is a Direct Hire Solicitation</p><h3>Major Duties</h3><p>Use clinical assessment skills to identify sponsors whose family members have special medical and educational needs in a timely manner.</p><p>Serve as the point of contact and subject matter expert (SME) for the Family Member Relocation Clearance (FMRC) program.</p><p>Collaborate with the Integrated Delivery System (IDS) and other interagency forums to provide family medical information and referrals to base and civilian agencies.</p><p>Provide clinical training to installation personnel and medical staff.</p><p>Assist in medical emergencies in the facility as needed.</p><p>Conduct quarterly case reviews.</p><p>Determine necessary resources and develops patient-specific assessments and plans of care.</p><p>Develop patient-specific assessments and plans of care.</p><h3>Education</h3>See Qualifications section for education requirements.
