Nurse (Clinical/Ambulatory)

<p>About the Position: This position serves as a Nurse (Clinical/Ambulatory) performing independent and comprehensive triage for patients, facilitate coordination with multi-disciplinary teams to provide the highest level of care, and provide education to patients. It is located at Womack Army Medical Center, Family Medicine at Fort Bragg, NC. This is a Direct Hire Solicitation Salary negotiation may be available for those candidates who are new to Federal service.</p><h3>Major Duties</h3><p>Perform independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach</p><p>Act as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention.</p><p>Screen patient charts to assess the medical history and determine health care needs.</p><p>Assist clinic with patient and staff education.</p><p>Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.</p><p>Develop appropriate nursing plans.</p><p>Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.</p><h3>Education</h3>See Qualifications section for education requirements.
