<p>Provides case management services to individuals and communities, ensuring coordinated care, compliance with public health & clinical guidelines, & evaluation of outcomes. Develops care plans & serves as liaison to patients, providers, & care teams. This position is located at the Chinle Service Unit, Division of Nursing Services, & reports to the Director of Communicable Disease Prevention and Control. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).</p><h3>Major Duties</h3><p>Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) - Coordinates case management for communicable diseases (e.g., STIs), reviews Lab results, monitors patient status, ensures treatment per guidelines, and initiates referrals and follow-up for patients and contacts. - Leads and coordinates contact investigations and outbreak responses, conducting field visits to assess, screen, treat, and manage individuals, families, and communities. - Plans, implements, and evaluates communicable disease programs; coLlaborates with Tribal, local, state, and federal partners; and coordinates outreach clinics and prevention activities. - Maintains disease registries, analyzes trends, tracks testing and treatment outcomes, and prepares required surveillance reports to support program effectiveness and public health action. - Provides education and training to staff, patients, and community partners; offers clinical guidance; leads care coordination efforts; and supports quality improvement and evidence-based practices.</p><h3>Requirements</h3>Selectee may be subject to a probationary period U.S. Citizenship is required Selective Service Registration is required for males born after 12/31/1959 This is a competitive vacancy announcement advertised under Direct Hire Authority. Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment.
