<p>Join the Indian Health Service at the Wewoka Service Unit and care for diverse American Indian and Alaska Native communities. This position functions in a case management role, providing care to patients across the continuum and collaborates with multidisciplinary health care professionals and community agencies. Coordinates services, health promotion and counseling, disease prevention, health education and screening, and community resources linkage for patients. Applies clinical criteria to monitor and facilitate in house consultations and Purchase Referred Care (PRC) to specialists outside of the facility. Manages aspects of the planning process in anticipation of patient variances or transitions. Develops an individualized comprehensive plan of care in collaboration with physicians and multidisciplinary team.</p><h3>Requirements</h3>Selectee may be subject to a probationary period U.S. Citizenship is required Selective Service Registration is required for males born after 12/31/1959 This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children.
