<p>The National CARDIO-VET Network is recruiting for a Nurse Care Manager (Registered Nurse) positions. Their are more than one vacancy and the possible duty locations for placement are: Danville, IL; Lafayette, IN; Marion, IN; Fort Wayne, IN; Togus, ME; or Bangor, ME. The incumbent will function as a Care Manager responsible for all elements of the nursing process when providing and/or supervising direct care.</p><h3>Major Duties</h3><p>The duties of the Nurse Care Manager (Registered Nurse) are but is not limited to the following: Assesses, plans, implements, and evaluates care-based age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and Veteran satisfaction through the continuum of care. Follows procedures per establishes policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Requires travel to multiple duty stations within the Cardiology Hub and Spoke Network. Executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. Provides patient management, care coordination, and discharge/disposition planning. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex cardiovascular care needs. Coordinates care with multiple providers across the Cardiology Hub and Spoke Network. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients. Addresses psychosocial, as well as nursing and medical needs of Veterans and their families/caregivers, through participation with the interdisciplinary care team. Coordinates and expedites care from diagnosis to treatment. Analyzes and manages laboratory and radiology results, and provides information to the providers. Utilizes databases to provide population management to track patient care through all episodes of care. Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet Veterans care needs. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve care for a group of Veterans. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, and quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for documenting broad and complex outcomes at the program or service level that can be demonstrated at any organizational level within a facility. You may be expected to travel for this position up to 75%. The position will require the operation of a government vehicle to travel to Community Based Outpatient Centers (CBOCs) to conduct outpatient visits. A valid driving license is required. Telework: Ad-Hoc only.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
