Mental Health Case Manager

<p>About the Position: This position is with the Defense Health Agency, Defense Health Network East at Martin Army Community Hospital located within the Department of Mental Health at Fort Benning, Georgia. This is a Direct Hire Solicitation</p><h3>Major Duties</h3><p>Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.</p><p>Develop, implement, communicate and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.</p><p>Communicate with treatment facilities, both in direct and purchased care to coordinate patient care and transitions. -Communicate/coordinate with appropriate parties including command and other medical teams within the guidance provided by DHA.</p><p>Track client progress towards goals, modify care plans as needed, and advocate for clients' rights and preferences.</p><p>Coordinate meetings to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical, Substance Use Disorder Clinical Care, Family Advocacy Program, and all other medical treatment providers.</p><p>Monitor psychotropic medication use, track physical and emotional side effects, recommend adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.</p><p>Schedule and monitor medical appointments, therapy sessions, and support services and ensure continuity of care across multiple systems.</p><p>Conduct regular resource and community needs assessments in order to be more informed about needs of client base.</p><h3>Education</h3>FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
