<p>The Licensed Practical Nurse - Patient Aligned Care Team (PACT) carries out the most complex practical nursing care assignments, which include performing nonstandard and specialized activities sufficient to solve a range of complex nursing problems, under the general review of a Registered Nurse in the outpatient clinic. The LPN uses a systematic problem-solving process in the care of multiple patients with predictable healthcare needs to provide individualized, goal-directed nursing care.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Duties include but not limited to the following: Demonstrate knowledge of practical nursing care exercising sound judgement in planning and intervention, requiring supervision to provide a practical knowledge of human body structure, sterile techniques and procedures. Care for Veterans in accordance with established nursing procedures, standards of care, and plan of care. Demonstrate technical competence to prepare, administer, and appropriately document Veterans education of medications, and nursing actions taken for commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Demonstrate the knowledge and ability to recognize the need for and to institute emergency measures when indicated. Demonstrate technical competence in setting up for Cervical Cancer Screening when assigned to a Women's Health PACT. Acts as a chaperone during women's preventative care screenings. Manage clinic workflow ensuring intakes are conducted in a timely manner and providing direction to patients as they await their appointments. Schedule appointments for Veterans in collaboration with team members. Perform and participate in Telehealth activities which may include VA Video Connect (VVC) Store and Forward, Clinical Video Teleheatlh and/or Home Telehealth Modalities seeking guidance and assistance when appropriate.
