<p>The Ambulatory Care Licensed Practical Nurse (LPN) GS-7 functions as a member of the health care team by demonstrating an exceptional degree of practical nursing knowledge and skills necessary for self-direction in carrying out complex practical nursing care assignments within the Community Based Outpatient Clinic (CBOC) or a Veteran population with a variety of health care needs. *This position is located at the Winchester Community Based Outpatient Center.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional The Ambulatory Care Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) GS-7 collaborates with a PACT team to provide complex/specialized patient-centered care to a panel of approximately 1200 patients taking into consideration patients' ages, cultural backgrounds, education level, and any other special needs. The LPN completes assigned duties and responsibilities that involve performing non-standard and specialized practical nursing activities requiring board nursing knowledge, experience, and skills to address a wide range of care needs. The LPN at the GS 7 level must exercise independent judgement and requires minimal supervisory oversight. The incumbent is expected to participate to a greater degree in prioritizing and, as appropriate, modifying patient care treatment plans. He/she must also possess the ability to observe subtle physical and emotional changes in patients, exercise sound nursing judgement, and demonstrate appropriate action based on those observations. The LPN must possess the knowledge and skill to promptly recognize potential urgent/ emergent patient care situations, seek team assistance as appropriate, and effectively intervene to assure the delivery of safe patient care. Duties include, but are not limited to: Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent situations. Demonstrates the ability to independently adjust or modify care being delivered to meet the changing needs of the patient with notification to the RN or provider for concurrence. Assesses, records, and reports vital signs, neurological signs, weight, and blood glucose levels. Appropriately triages and prioritizes patient needs as routine, urgent, or emergent in the clinical setting and modifies patient care/treatment as needed. Completes all required documents in a timely manner in accordance with VA policy and TJC standards of care. Demonstrates excellent written and oral communication skills. Prepares, administers, and documents medications and immunizations as directed without error that may cause harm to patients. Demonstrates practical and theoretical knowledge about medication side effects, purposes, therapeutic dosages range, and toxic effects with skill sufficient to administer a wide range of medications. Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
