<p>The VA Northeast Ohio Healthcare System recruiting for an Licensed Practical Nurse. The Licensed Practical Nurse will function within the Telehealth section of the Community Outpatient Services.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Provides nursing care for adults with population specific needs by incorporating pertinent physical, psychosocial, and cultural needs into the patient plan of care. Promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Demonstrates nursing practice with remote supervision of a RN, physician, or licensed independent provider. Collaborates with and support the RN, physician, or licensed independent provider with appropriately prioritizing the overall patient care needs and adjusts plans for care delivery pm. Proactively assists the RN, physician, or licensed independent provider in addressing patient's needs for medications/treatments, accurately preparing and administering medications/treatments accurately and promptly under the general supervision of a RN, physician, or licensed independent provider per policies/procedures and including the patient's response to the medication/treatment Demonstrates understanding of the effects and/or subtle signs and symptoms of potential reactions caused by medications, treatments, and procedures. Promptly records and reports these observations and initiates appropriate, prescribed interventions. Performs personal care, diagnostic support duties and treatments according to procedure with minimal direction from the professional nurse in a timely manner. Completes required patient care documentation and records significant observations and actions utilizing the nursing process in a clear and concise manner. Demonstrates the ability to assist RN, physician, and/or licensed independent provider to appropriately prioritize overall patient care needs and adjust plans for care delivery. Makes astute observations of subtle, less obvious changes, both physical and emotional, in patient's condition, and accurately and thoroughly documents care delivered and patient's response to care that could require immediate modification of the patient's care plan. Demonstrates the knowledge and skill to recognize potential urgent/emergent patient care situations, seeks assistance of the RN, physician, or licensed independent provider for team interventions and effectively intervenes, as directed to assure safe and therapeutic patient care. Supports the RN, physician, and licensed independent provider in monitoring the practice of unlicensed or non-medical staff in patient care delivery. Promptly reports care that deviates from normal practice patterns to the registered nurse, physician, and independent provider so that care issues may be addressed immediately Performs Telehealth imaging, including Tele-Eye screening (Teleretinal Imaging and Tonometry), Teledermatology, Telewound care and other Telehealth programming, as identified and referred by the primary care provider, other providers, and the imager. Provides patient education regarding the imaging process. Facilitation of the clinical, business, scheduling and technical aspects for real-time Telehealth applications, including specialty services and VA Video Connect patient education. Conducts quality control procedures on equipment and products and maintains proper records for quality control reports and workload reporting. Coordinates with Biomedical and Information Technology staff for the installation upgrade of hardware and software for all Telehealth technologies.
