<p>This announcement is recruiting for Licensed Practical Nurse's (LPN) for the Beaver County Veterans Affairs Outpatient Clinic with an anticipated start date in late 2026 or early 2027.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Duties include but are not limited to: Main responsibility is daily management of the Primary Care Provider's (PCP) scheduled appointments and clinic flow. This is accomplished through assisting PCP with preventative care and/or chronic disease management by obtaining vital signs, offering patient education, screens Veterans for a variety of preventative maintenance or chronic disease care needs, manages immunizations, injections and all other ordered medications, assists with exams/procedures during the visit such as pelvic exams, joint injection, wound care, Teleretinal, Telederm, EKG's, Phlebotomy, bladder scans to name a few. Other responsibilities includes assisting the PACT team or other interdisciplinary team members with panel management and care coordination. Such tasks include test result notification, patient education, discharge follow-up, and chronic disease management. Serves as an active participant in planning, implementation and evaluation of care. Participates in daily huddles and team meetings with PACT team members. Demonstrates, in interactions, the ability to communicate effectively and professionally with both internal and external customers. Is knowledgeable of and properly initiates nurse protocols and Standard Operating Procedures (SOP) to help deliver holistic care. Confers with LPN TL, RN Care Manager, PCP, or appropriate person to deliver care when needed. Uses motivational interviewing and health coaching to guide patients in achieving goals of health promotion and disease prevention. Participates in panel management: Completes pre-clinic phone prevention up to 24 hours before scheduled appointment. Assists with chronic disease management and other care coordination needs. Use of telehealth technology to render care. Completes view alerts assigned within 48 hours. Provide New Patient Orientation when you encounter new patients. (PC Booklet) Completes quality control checks on testing and other equipment/supplies. Stocks as needed. Informs the CN, LPN Team Leader or Nursing Leadership of broken equipment needing repair. Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, or supply deficiencies and reports to appropriate personnel for corrective action. Participates in performance improvement and quality improvement intiatitives. Serves as LPN Team Leader (TL). Works with the CN to facilitate clinic operations. When assigned to the LPN TL role the LPN TL will: Prepare daily assignments, including breaks and lunches and other tasks as appropriate by 7:45am daily. Ensure completeness of all quality checks prior to the start of clinic. Replenishes stock order for medication and supplies as needed. Ensures that assignments are properly delegated, clinic flow maintained, provides feedback to fellow LPNs and Unlicensed supportive personnel such as Healthcare technicians and clerks as needed and reports to the CN or designee any information needed to enhance and facilitate clinic operations Acts as a resource to fellow LPNs and Unlicensed supportive personnel such as Healthcare technicians and MSA's. Preceptor or mentor students or new employees. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
