<p>This Licensed Practical Nurse (LPN) Patient Aligned Care Team (PACT) is located in the Primary Care department at the VA Boston Healthcare System, Framingham Community Based Outpatient Clinic. This position is full-time at 40 hours per week.</p><h3>Major Duties</h3><p>VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional The Licensed Practical Nurse (LPN) is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan of life. The LPN is a member of the Patient Aligned Care Team (PACT). The LPN demonstrates active collaboration and communication with the team and may direct other members of the team in providing certain aspects of care for a designated panel of patients (patient, primary care provider, registered nurse care manager, clinical and clerical associate) and expanded team (family caregiver, internal and community based services and disciplines involved in providing care to the patient). The LPN reports to and works under the supervision of the Primary Care nurse manager or operations manager in Primary Care for the designated site or program. Work Schedule: Monday - Friday, Day Shift (hours to be negotiated with supervisor). Permanent Change of Station (Relocation Assistance): Not authorized Compressed/Flexible: Available Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA. Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework. Ad Hoc as determined by the agency policy. Defined as telework that occurs on an occasional, non-routine basis. Telework that occurs to complete short-term special assignments or to accommodate special circumstances even though the telework arrangement may occur continuously for a limited and specific period of time.
