ABOUT THIS ROLE
<p>The primary purpose of the Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. The LPN/LVN routinely travels with inpatient, contract nursing home patients and outpatient Veterans who require assistance with activities of daily living.</p><h3>Major Duties</h3><p>Total Rewards of a Allied Health Professional The LPN is the Patient Care Aligned Team (PACT) teamlet member who provides clinical support for the delivery of primary care services and operations management to a PACT. The LPN also is responsible for providing evaluation and care consistent with licensure, certification, and functional elements of practice, to patient assigned PACTS's. The LPN collaborates with PACT staff to develop comprehensive health care plans and care management plans for patients assigned to patient plans. The LPN manages clinic workflow by ensuring patients are placed in examination rooms in a timely manner and providing direction to patients as they move through the clinic environment. Duties include but are not limited to: Assist with triage as appropriate. Assist with exams/procedures. Assist with group visits as directed. Set up supplies, prep room for procedures. Perform treatments (VS, EKG, FSBG, ear lavage, toenail care) Administer meds, performs wound care. Complete pertinent clinical reminders Assist with medication reconciliation. Assist with post discharge calls as directed. Track and administer required immunizations. Run injection clinics Work Schedule: Monday-Friday 7:00am-3:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
