<p>The Licensed Practical/Vocational Nurse, GS-06 is a member of the healthcare team, who performs a variety of nursing skills specific to the clinical needs of patients on the assigned work unit and under supervision of a registered nurse in a variety of inpatient and outpatient locations.</p><h3>Major Duties</h3><p>This position will provide individualized patient/resident care in relation to the following CORE concepts; Safety, Technology, Program and Services, Equipment, Documentation, Communication and Philosophy along with established Position Specific Competencies. LPN will display skills, knowledge and commitment necessary to comply with all established procedures, guidelines and SOPs found in CORE concepts and Position Specific Competencies. The duties are as follows; but not limited to: Demonstrates knowledge and skills to carry out a full range of practical nursing care and specialized patient care duties to patients with a variety of physical and/or behavioral problems in the outpatient setting. Assists with patient education and patient groups regarding medications and disease management. Re-enforces all instructions previously given to patients by the MD/PA/NP/RN. Demonstrates ability to manage telephone care to include disease management, medication management, lab and radiology results and other telephone communication. Utilizes nursing protocols with attention to the individuality of patients, patient safety and evidence-based practice.
