LPNs administer care and apply nursing intervention techniques to patients with a wide range of disease entities. The incumbent must be able to promptly and accurately detect, identify, and report significant changes and/or responses to treatments or unusual circumstances using the facility recommended communication tool to improve patient safety and the transfer of critical information. LPNs must be prepared to cope with unpredictable behavior problems related to or in conjunction with the geriatric or dementia patients. The incumbents work assignment consists of providing acute, intensive and emergent nursing care to patients in the geriatric or dementia care area, preventing the occurrence of complications and/or life-threatening situations, and providing optimum care while functioning as a team member under direct supervision of the professional nurse. Contacts are with patients, families, and all members of all health care disciplines in the geriatrics or dementia areas of the medical center. The purpose of the contacts is to provide quality and continuity of nursing care in the geriatric or dementia care setting. The setting demands providing care, emotional support, and instruction for the patient and family. An overall supportive environment should be provided to aid in the patient's recovery. The employee is involved in long periods of physical activity in the geriatric and dementia areas, including standing, bending, lifting, stooping, stretching and the pushing of beds, stretchers, wheelchairs and other equipment. Nursing care duties are performed in the geriatric area Community Living Center(s) (CLCs). The assignment involves contact with and use of specialized equipment. LPNs are expected to practice safe work habits. The incumbent is exposed to stressful situations on a daily basis due to unpredictable conditions and occasionally unpredictable resident behavior within the CLC neighborhoods. LPNs will be exposed to infectious diseases and handling of contaminated and hazardous waste materials (soiled dressing, contaminated needles, syringes, suctioning and testing equipment, etc.) and use of isolation techniques. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
