Job Description:

• Complies with all department policies, procedures, practices, and standards of care.
• Assists the physician and home health team in understanding significant social and emotional factors related to health.
• Performs home assessments to evaluate potential social needs and conducts visits to implement a plan of social service assistance.
• Counsels patients and family members to develop attitudes and behaviors supportive of maximum health practices.
• Utilizes community agencies and alternative resources to assist patients and families in resolving unmet health care needs, financial, and environmental difficulties.
• Reports pertinent observations to the case manager/supervisor and attending physician.
• Participates in the team process through cooperative efforts with team members in developing the patient care plan and team conferences.
• Provides comprehensive, individualized care to assigned patients.
• Supervises BSW degree social worker(s) and co-signs patient documentation completed by BSW.
• Evaluates BSW(s) performance, abilities in care planning, and effectiveness by patients' response.
• Evaluates patients as assigned.
• Other duties as assigned.

Knowledge, Skills, and Abilities:

• Demonstrated knowledge of medical social worker modalities, treatments and skills. [Required]
• Mental health counseling, services to the aged/ill. [Required]
• Broad base of available private and government resources. [Required]
• Problem solving abilities. [Required]
• Positive interpersonal communication skills. [Required]
• Team and customer service oriented. [Required]
• Basic computer skills required. [Required]

Education:

• Master's [Required]

Field of Study:

• Social Work

Work Experience:

• 1+ year of social work experience in a health care setting [Required]
• 2+ years of home health social work experience [Preferred]

• Clinical Social Worker License (LCSW) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]

Pay Range:

$26.08 - $48.50
