Please learn more about the position below, along with the duties.</p><h3>Major Duties</h3><p>Two Full time position exist for an experienced registered nurse in the Geriatric Extended Care (GEC) Home Based Primary Care (HBPC) Program This position reports to the Nurse Manager of Home-Based Primary Care. The HBPC RN is a member of the interdisciplinary team providing direct primary care to a specific group of homebound patients with a variety of chronic and terminal illnesses. The RN is responsible for Care Coordination on an assigned caseload utilizing professional nursing skills in collaboration with the interdisciplinary team members led by the licensed independent provider. Responsible for implementing the home health plan that meets each Veteran's specific need that complies with the organizational standards and federal and state regulations, and the VAMHCS policies and procedures. Participates in weekly Interdisciplinary team meetings (IDT) and in the performance and continuous quality improvement activities and initiatives. May oversees the nursing care of Veterans receiving care in community- care sponsored or contract programs. Duties include, but are not limited to: -Collects current symptoms, as well as a detailed patient history and then consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans. -Performs triage, while considering both physical and psychosocial elements. -Nurses in this role provide care to a caseload of Veterans in the home setting. The service area for HBPC is considered within a 40-mile drive radius from the Central Virginia Healthcare System (CVHCS) main campus, Community Based Outpatient Clinic (CBOC), or telework location as applicable. -Demonstrates the ability to complete in-home assessments including the Veterans risk factors; physical, psychosocial, emotional, and mental status; medications; home environment and safety; caregiver/support system; and educational needs. -Develops an initial plan of care with the patient and caregiver. -Based on the assessment, referrals are made to other members of the HBPC team that includes Provider, Social Work, Occupational Therapy, Dietitian, and Psychologist. -Reviews the interdisciplinary plan of care with all team members during the weekly team meeting. This plan of care is reviewed every 90 days during the team meeting, or as needed based on changes in the patient's condition. The review note is co-signed by the provider. -Collaborates with HBPC pharmacist through interdisciplinary team meetings, or as needed based on quarterly pharmacy review of medications. -Consults with inpatient discharge coordinators, nurses and social workers to facilitate the transition to home care. -The nurse reviews the medications the patient has in the home to ensure they are correct and match the current orders from VA physicians. The nurse educates the patient regarding their medications and provides a current medication list for use in the home. -The HBPC nurse also assesses for any over the counter medications or supplements the patient may be taking and conveys this information to the primary provider for further evaluation. -Operates a government vehicle for travel to home visits.
