General Description The Counselor II is a valuable member of our (ACT) Assertive Community Treatment Team to provide intensive case management for adults with chronic, severe mental illness. Our team works to reduce hospitalizations, avoid nursing home placement, ensure basic needs are met, maintain engagement in treatment, and promote harm reduction.
