<p>The VA Northeast Ohio Healthcare System is recruiting for an Compact Act Registered Nurse. The Registered Nurse will function in the Patient Transfer Center (PTC) section of the Patient Care Administrative Service (PCAS).</p><h3>Major Duties</h3><p>Duties of the Compact Act Registered Nurse include but are not limited to: Delivers fundamental knowledge-based care coordination to assigned clients. Demonstrates knowledge concerning the care and movement of patients throughout the facility and non-VA facilities. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Influences care outcomes by collaborating with members of the interdisciplinary team. Conducts chart reviews, ensure obtainment of medical records, and document care needs in Electronic Medical Record (EMR). Assist with Veteran needs assessments, identification of barriers, and activities required to successfully transition the Veteran to and from an inpatient/residential settings. Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgement to ensure positive patient outcomes. Coordinates care with multiple providers across all levels and sites of care. Assures services interface within the VA and community programs to provide continuity of care to patients. Collaborates with other VA and non-VA departments, community agencies, and healthcare providers to identify barriers and activities required to successfully transition and ensure Veteran needs are addressed and discharge planning is comprehensive. Assists in reviewing a care coordination plan, reviewing medical documentation from community providers, communication of care coordination needs, and supporting the Veteran through the completion of the episode of care. Influences care outcomes by collaborating with members of the interdisciplinary team both within the VA and in the community. Complies with VA-mandated processes and use of health information solutions that serve to assess, document, exchange, and track care coordination information. Utilizes delegation of authority, if applicable, to function at their highest level of role performance and align high quality care with key performance indicators to ensure practice standards meet Veteran care needs.
