ABOUT THIS ROLE
<p>The Community Care Nurse Navigator focus is on individual growth and development in practice with increased levels of self-direction. Provides patient management, care coordination, and disposition planning for Veterans in the community. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.</p><h3>Major Duties</h3><p>The Community Care (CC) Nurse Navigator (NN) Registered Nurse (RN) has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Provides patient management, care coordination, and disposition planning for Veterans in the community. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to provide continuity of care to patients. Provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the overall treatment plan to include other duties but are not limited to: The Nurse Navigator supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services. The Nurse Navigator will interface with outside referral agencies and ensure that their business and clinical practices are in compliance with regulatory standards. The Nurse Navigator will addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Reports status of service needs and contributes to problem resolution and modification of processes as necessary. The Nurse Navigator demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. The Nurse Navigator has knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines. The Nurse Navigator uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their care in the community. The Nurse Navigator supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services.
