<p>Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established polices and guidelines.</p><h3>Major Duties</h3><p>The functions and duties of the CNH/COMMUNITY HEALTH NURSE REGISTERED NURSE include but are not limited to the following: Contract Nursing Home (CNH) RN provides nursing expertise in the CNH program. The RN provides CNH resident oversight, consultation, and case management. The RN is a vital team member along with the CNH Coordinator, CNH Social Worker, and CNH Medical Director. Conduct site visits to the contract nursing homes; audit charting, interview Veterans regarding their care, ensure quality care is being delivered. Identify patient care needs in accordance with the established standards, policies, and procedures consistent with nursing practice. Interacts with Veterans, family members, and an interdisciplinary team of professionals. RN must follow up on referrals, consults, medical needs, and other tests to ensure continuity of care for Veterans residing in CNH homes. RN must monitor for appropriate utilization of care by conducting routine inspections of community nursing homes that have a contract with the VA. The RN must document CNH visit notes and other case management needs per VA guidelines. The RN must have an active driver's license and be able to sustain long periods of sitting and driving. and may be exposed to weather such as tornadoes, rainstorms, snow, ice, and other various weather related elements. The RN may need to develop creative and innovative strategies to resolve problems and/or promote positive outcomes in collaboration with other CNH team members. RN must monitor for appropriate utilization of care by conducting routine inspections of community nursing homes that have a contract with the VA. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans. Relocation/Recruitment Incentives: Authorized to highly qualified and eligible new federal appointees or former federal employees with at least a 90-day break in service. Permanent Change of Station (PCS): Not authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
