Clinical Pharmacy Specialist (Pain Management)

<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The duties of this position include, but are not limited to: Responsible for the development of a therapeutic plan which is intended to achieve definite drug-therapy outcomes that improve the patient's quality of life Interviews patients to determine medical history and reviews history for potential drug interactions, poly-pharmacy, therapeutic duplication, noncompliance and adverse drug reactions Assess the need for risk reduction related to opiate therapy including dose, signed pain contract, recent UA, and benzodiazepine co-administration. Works with patient and providers to maximize benefit and reduce risk Monitors and manages medication therapy under an approved scope of practice with an emphasis in pain management Provides drug information to physicians, nurses, trainees, and other health care professionals Participates in interdisciplinary care conferences related to the Pain Management program Supervises and precepts pharmacy residents and students as assigned Work Schedule: Monday-Friday, 8:00am to 4:30pm Telework: Not Available Virtual: This is not a virtual position.
