<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>Duties include, but are not limited to: Optimizes medication therapy using pharmacokinetic/pharmacodynamic principles and collaborates with prescribers on therapeutic decisions Performs patient-specific therapeutic drug monitoring; communicates recommendations to providers verbally and in writing. Reviews medication regimens for appropriateness, duplications, interactions, contraindications, and clinical effectiveness. Conducts comprehensive medication counseling and educates patients on proper medication use. Completes medication histories and medication reconciliation across transitions of care. Provides evidence-based drug information to prescribers, interdisciplinary staff, patients, and caregivers. Reviews charts, laboratory data, and clinical indicators to monitor drug efficacy, safety, and adherence. Dispenses medications accurately, verifying orders for correct drug selection, dose, route, and documentation. Resolves medication-related problems by communicating with providers and recommending alternative therapies when appropriate. Supports outpatient prescription processing, unit-dose activities, and complex therapies in coordination with care teams. Completes Prior Authorization Drug Requests (PADRs) and recommends formulary alternatives when needed. Participates in quality assurance, MUE programs, adverse drug event reporting, and medication safety initiatives. Manages recalls, drug shortages, and therapeutic interchange processes in accordance with policy. Provides oversight and guidance to pharmacy technicians; precepts pharmacy trainees and contributes to staff development. Maintains professional competency through continuing education, literature review, and adherence to pharmacy best practices.
